Will 3 LOR hurt by not having 4?

Most ACGME IM programs have 3 LOR required with 1 being from the Chair of IM, as well as the Dean's letter.

I only have 3 thus far, and after bugging the 4th LOR writer's office for months now, there's still no developments with ERAS "deadline" next week. Will I be at a significant disadvantage by not having an extra 4th letter? My letters now are from an IM/Peds doc, a sub-IM specialty, and the chair letter :/
