WL at two schools, can I write to both saying that they're my first choice?

Even if you do get accepted to both on the same day, what's the worse that could happen? One school will think you're full of **** but that doesn't matter cause you won't be going there.

It might not be the most upstanding thing, but no school seems to feel bad about taking your application money and sending a rejection 6 months later.

Yes, it does. Remember medical school Admission offices are only a phone call/email away from each other.
 
Unless you've somehow managed to figure out how to have two first choices, I'd classify this as dishonest.
 
Some schools don't care about letters of intent. For these, feel free to send them one.
