Should i put the company name, or the actual person who employed me? I am running out of room since I put all my tutoring activities into one field (limit of 5 working experiences) >___<

I put company name, but I worked for large retail businesses.

If it is a small business/practice putting down the name of the person won't tell them much. I would much rather see like "Happy Smiles Dentistry" as employer than John Smith.

Is there an area for contact phone numbers / emails so they can reference your past employers? I really want to have one of old bosses write me a letter but I think it would help me more to have a dentist write it and put the boss down a reference in my work history. Is there a place to write addresses/contact info for past employers or names of supers? Can I count on them contacting them or should I get a letter to make sure?

I do not think they would contact past employers. Takes too much time and is not useful to them. There is not a place to enter a phone number, unless you put it in the job description box. It only asks for name of company, position, hours, start and end date, and a job description.

What kind of job was it? Was it just a retail job or something important?

Some schools state they want a dentist letter, but will take letters from mentor, or places you volunteered at, bosses, etc.
Others only say they want a dentist letter and do not say if they will take a letter from someone else to substitute it. Not sure how strict they are on this.
