Those that are WRVU driven pay, do you have problems with your facility not calculating your wrvu correctly? So, far over 6 months, my calculation has been about 20 higher per month. However, they have a new work que for billing that is holding up my charges related to covid screening and now halfway through the month I am missing already 40. My practice manager basically gives me lip service and I imagine I will be down around 100 at end of the month or more. Is it common for WRVU not to be counted correctly? Before the new que, I chased every charge daily and kept up with it last month and charges were all there. So, I know I am doing my charges correctly. But now with several ques my charges can be held up in there is no way for me to watch it closely. We have EPIC. I mostly like my job and pay is good, but I cannot help but pissed off with so much missing in terms of WRVU. I have already pissed off the practice manager when the new que started and she had no clue where the charges were going until I basically told her she needed to find them or I was going to her boss or higher. Upper management does not seem to care if practice is not billing all it can and especially if it is messing up my funds. What to do? I tried asking nicely to practice manager to find the charges and she does not reply to emails or texts. Unless I threaten to go to higher chain.

Are you paid by wRVU billed or collected? If collected, there will be a lag so you cannot reconcile your number and their number quickly.

Practice manager may not know the answer. My guess she's busy and you're adding more work on her plate. Then she's probably feeling you'll get her in trouble for not doing the extra work. Go higher up for the answer.

If I have a question about wRVU, I ask the head guy through e-mail and he has been responsive.

For me, I anticipate a bit of slippage where my work isn't being counted or collected. I view that as charity work -- giving back to the community. I know my employer well enough to trust the people there are not trying to screw me over. However, the reported wRVU falls within my anticipated range. On the other hand, + / - $50k is a rounding error to me.

If you don't trust your employer, I can't see you lasting there. Speaking from experience.

+/- 50k is a rounding error? Even if you were making 500k that would still be 10 percent of your income, so does that mean you’re making 1 mil per year where it would be about 5% of your income? Still not really a rounding error unless you get to 2M+

Is the 20 per month just lag from the last day or two of the month? Our hospital billing software lets you run a report of all RVUs so our count can be checked. Same software the billers use so there’s no confusion about with different computer programs.

I believe the some of the missing wrvu were carrying over to next month. I did not know that at first. This new que has changed everything for me as I cant track down something missing easily. I am billed. It will all end when we stop screening for COVID. So it is not permanent. This really only affect inpatient and specifically me so other MD providers are pretty much not touched. And it is almost all my work. It is really starting to affect my attitude at work when I feel like I am being ripped off. I will give it the full month before I go in on higher management. But they all are friends and been there years. Guess I know why nobody stays despite very good pay. They treat you like an outsider.

I see. Why wait a full month? Reach out to someone higher up. This is obviously very important to you as you are uniquely affected by this. If I remember correctly, considering you supervise NPs, administration has bigger incentive to make you happy by giving you answers.

LOL. Been where you are, done that, got the T-Shirt. I have nothing constructive to add. G'luck.

Epic does this for me (outpatient FM). I was told that since all the RVUs get counted eventually that they didn't really worry about it.

Just wait until the billers or some one behind the scenes starts subtracting wRVUs for various reasons. Or the Big Box shop creates their own wRVU values separate from standard CMS numbers, or they simply say that they won't count these codes for your wRVUs because they don't get paid so therefore we won't count it.

Queue.

And I wouldn't back down, this shouldn't be happening.

If it only cost < 10k/yr and you like your job, I wouldn't bother if I were you...
