If you are gung ho on neuroIR, look into programs with in-house neuroIR fellowship that historically take neurology applicants and start building connections/research beginning even in PGY1.

I see, thanks for the advice! Do you have a rough idea of what kind of schools I am competitive for/what I should use my signals on?

You will have a good chance with most programs being USMD with good score and research. The last factors are your LoR and performance during interview. Signals are very personal decisions, and oftentimes people have geographic preference and would then send signals accordingly. I would recommend starting with geographic regions where you would like to go and then narrow down the list based on each program's strength (in your case, do they offer NIR or has any recent graduate matched NIR).
