I dunno about Cornell, but I think MGH >>> Columbia. I think you should decide first if you want to live in Boston (--> MGH) or NYC (--> Columbia/Cornell). Then if you want to live in NYC, decide between Columbia and Cornell. If you really want to do peds, maybe Columbia would be better, but otherwise I would pick Cornell. I think Cornell is overall better than Columbia - similar reputation, but better location and housing.
