Out of curiosity... How many interviews do you guys feel you need to go on to feel pretty comfortable about matching? I've read 3-4 for exceptional candidate, 5-7 for average, and 7-10 for below average. Is that still the consensus because it's seeming more competitive after speaking with candidates from previous years?

I agree that it's more competitive. A few applicants last year who thought they were competitive did not match. I have no idea how many interviews to feel comfortable.... as many as possible.

Witty Fool and I just went through this last year, and there was something like a 30% unmatched rate for US grads applying. This is a very competitive fellowship.

As far as number of interviews to feel comfortable, the advice I got was:

1. Competitive applicant (US MD, Anesthesia > Pm&R >>> Neuro > Psych) with a home fellowship where you are well-liked: 6-8 interviews
2. Competitive applicant with no home fellowship: 8-10 interviews
3. Everyone else, especially those from Neuro/Psych background: As many interviews as you can get

Remember that many of these programs advertise 4-6 spots but at least 50% of those will likely be taken by in house candidates (even if the program denies this on interview day). So when you look around the room and think that you are competing with 75 interviewees for 5 spots its more like 73 for 2-3. That's why it's really important- if you think about Pain early- to go to a program with a home fellowship.

Places like the Brigham will advertise 9 spots but probably 6-7 of those will be taken by either in-house folks or people from the other Boston programs.

Pain is also a small world and the top programs tend to take applicants from other top programs that have strong Pain fellowships, people involved heavily in pain research, ASRA heavyweights etc. If you are coming from a mid tier program without a track record of putting people into Pain fellowships in recent years you are at a distinct disadvantage.

So consider all of these factors and then decide what a comfortable number of interviews is for you.

61N summarizes it well. Basically programs have a lot of interest in bringing on their own due to familiarity with the applicants. I will add that the bigger the name, the taller the wall - residents go there for the sole reason to match into their fellowship and thus make themselves extremely competitive. You're an outsider. They are not only insiders, but impressive insiders with access to the ears of their directors. The barrier to entry is higher.

The reassuring point that I will add is the awareness of many programs that pain is a multidisciplinary field, and are thus open to doctors from a growing diversity of specialties. More programs are coming to this awareness. The future of pain lies at the crossroads of disciplines. The field needs it, and patients deserve it. I met applicants who were PM&R and Neuro on the trail, in addition to Gas.

My best advice is to apply early, be yourself, and have some strong cards in your hands. Research, a rotation, a strong letter, a genuine interview, pull it all together.
 
I personally know people with 2 and 4 interviews that matched. I also know
Of a guy with 9 and at good places that didn't the first time around. Super unpredictable
 
I agree it can be unpredictable, but the percentage of residents that match with 9 interviews is definitely higher than those that only receive 2-4. If the individuals chose to only go on 2-4 out of numerous invites, that's a different situation as well.

Appreciate all the opinions though, it helps.

There are always anecdotes of people who didn't match who should have and vice versa. We all know pain is very competitive. Here is what you need to know...

There are 5 criteria to be considered a top-ranked candidate for pain (in order of importance):
1. INTERVIEW: A benign interview. You don't need to wow them but you should be someone that they think they can work with for 12 months. I understand that #2-5 land you the interview but if you're crazy during your interview, it doesn't matter how stellar the rest of your application is, you won't be matching.
2. RESIDENCY: An anesthesia residency at top-rated program (eg. MGH, BWH, Hopkins, Stanford, Penn, etc.)
3. LOR: Good letters of rec; at least one should be from someone relatively prominent in the field, the others can be from anyone who knows you well and are strong
4. RESEARCH: Some pain-related work; ASRA posters, book chapters, research, etc. (1-2 projects is sufficient, the more the better)
5. PS: a personal statement that makes you looks like a normal person
Note: ITE, USMLE, med school transcripts, advanced degrees (unless you did a PhD in something related to pain) don't play a big role.

If you have all 5 of these, I'd be confident with only a handful of interviews (3-4 interviews). I had a mix of these 5 criteria and matched at my top choice, which has been considered as one of the top programs in the country throughout this forum. The other 4 residents from my program also had stellar applications (ie. had all 5 criteria) and matched at their top choice as well. So arguably, we all could have applied to a single program and been fine. But no one is dumb enough to suicide match, no matter how talented they are.

If you are missing any of these criteria, you either need to:
1. Compensate in another area (more pain-related work, interview very well, have a LOR from a true leader in the field); or more realistically,
2. Interview at more places. The more you lack, the more interviews you want to get. Applying early helps. (ERAS opens in December, first interview invites roll out in February).

Overall, my advice would be to interview at as many programs that you are realistically interested in attending. It can be expensive but find the money (credit cards, borrow money, loans, etc.) No one regrets spending a few extra thousand dollars to ensure their future. But you can be sure you will regret not spending the money if you end up not matching. If you don't think you're competitive based on the above criteria, you better go to every single interview you are offered.

I agree with paintrain on what the programs are looking for. It seemed a lot of programs really where looking to fill with all or mostly all with their home applicants. Some probably try and take a least a couple outsiders, but they want their own program to look successful, so they will make sure their people match, especially if they have been a solid resident. Agree that a letter from someone people knows goes a long way as I am a PM&R resident from a good program that doesn't have an Pain fellowship or an academic Gas program. My letters were from our pain/interventional spine attendings, but I don't feel that they carried me as far as some people from lesser programs that had fellowships and letters from better known people. I still wound happy with where I ended up, but would advise anyone in a similar situation as me to apply broadly and go on as many interviews as possible. There were definitely a lot of really smart and great people that I met on the interview trail last year at both the accredited and unaccredited places I interviewed at so I only imagine that the competition will likely keep getting tougher.

Everyone says to apply "broadly". But what does this mean? There are 95 ACGME fellows, do I apply to all 95? Only about 8-10 (according to my research) "like" psych applicants. Am I wasting cash by applying to all 95? Obviously, there are fellowships that only look at Gas guys, so I will eliminate those, but that may still leave me with 75 programs...

Also, your thoughts on going to meetings? I am a member of AAPM/ASRA. Is it worth going to these annual meetings to try to make connections with PDs? I have had mixed responses on this. Obviously, I'm going to try to get an abstract/present a poster, but if this falls through, should I still try to hit up a meeting, or is it pointless?

Apply to your 10 that welcome multiple specialties, or so you think. Then pick another 20 that you have a special interest in and apply to those. If no invites crop up you can always add more as the season goes on as far as ERAS is concerned.

My post about the 5 criteria to be an ideal pain applicant generally applies to most situations. But in recent years, there has been a push for programs to teach a "multidisciplinary" approach to pain medicine. In as much, programs have welcomed, sometimes even sought out, the non-traditional applicant (i.e. PM&R, psych, neuro). A program that was traditionally all gas may decide they want to fill a spot for these non-traditional folk now. Don't be discouraged.

If you're desperate for pain and feel you are at a disadvantage, I'd apply to more than 30. ERAS has streamlined the process of applying to additional programs (an extra click of a mouse and ~$10 dollars?) I'd advise against applying to some now and adding more later. You never know when programs will review and fill their application spots. If you add a program later after they've already reviewed applications and decided on who to invite, adding additional programs at that point would be futile. You never know what a program may/may not be looking for but like the lottery, you can't win if you don't play.

To answer your question of "is it a waste to apply to all 95 programs," no one can say for sure without knowing your entire application. But in short, it would be a waste if your application is either stellar or hopeless. If you're stellar, over-applying is overkill. If you're hopeless, applying to 1000 programs probably won't make a difference. The benefit lies in an application that is slightly above/below average because for these, there is an inherent "interview hit:miss ratio" that depends on what certain programs are looking for. Unfortunately it's hard to tell exactly what a program is looking for (besides the obvious 5 criteria that we discussed) so while applying broadly will definitely increase your "misses," the hope is that it will also increase your "hits."

Go to meetings to present and to add something to your CV. Mingling with PDs won't make/break your application. It's icing on the cake. But if you have a lousy cake (i.e. application), no amount of icing is going to help.

No joke. I thought it would pick up towards the end of April.
Hang in there. There are about 50 programs which have yet to offer interviews. The core of it in May and June, the remainer in July. If you take a look at the threads from 2014 and 2015, you'll have an idea of who offers when (although some programs seem to have taken an earlier start this year). Also, several program coordinators have written back to my confirmation requests with specific timelines, and many don't finish reviewing applications until the end of May. In fact, it seems that some don't even look at them before that date. So... hang on.
