Clinical as a doctor: Nothing until he writes the USMLE + some sort of GME (residency) for a minimum of 2-3 years depending on thestate.
Clinical as a non-doctor: there are some accelerated mid-level programs (nursing, PA) for foreign trained doctors. Will still require some years.
Non-Clinical: The only formal requirement is a work visa. But obviously job-specific. Try googling "non-clinical jobs for foreign medical graduates."
But I'm not an expert. Forum search will give more detailed answers.

It is highly unlikely that any residency program is going to take a 50+ year old foreign physician, so these accelerated transition programs are probably his best (and only) bet if he wants to work clinically.
