Applying in peds ...without a rotation in CA1?

No. Many programs do not have CA-1's doing peds. In fact, it's almost certainly the majority. And you don't apply until you're a CA-2. You should have plenty of time to complete a peds rotation before your application.
 
as the other poster mentioned, almost no programs have you do peds as a CA1 (there are a few exceptions). There is no point IMHO as you gotta master the basics first.

And as with residencies not expecting you to know anesthesia, fellowships shouldn't expect you to be amazing in their area. That's what their year of additional training is for.
