Are practice test scores indicative of the real thing?

I am taking the MCAT tomorrow for the first time and I am a little nervous (and by that I mean it feels like my stomach is going to fall out of my ass) I took a practice test yesterday and got a 28. When I take partial practice tests, I can get higher scores. The highest I have made is 11 in verbal, 10 in physical sciences, and 11 in bio. Does added focus come on the test? Yesterday I missed about ten questions because I did not read the question correctly. Do you become less nervous when you are actually taking the test? If anyone has taken the MCAT and has advice please let me know.
