What is the difference between CNA and GNA? If I want to get clinical experience as a CNA in a nursing home, do I have to become a GNA (Maryland)?

In Maryland you need to take additional licensure testing to get a GNA. CNA certification requires no licensing test, and you are correct in that it is more suited to hospital work vs nursing home work where a GNA is preferred.

Tbh being a CNA in the hospital will be a better option. You'll have more contact with Dr's and you'll get a better idea of everyone's roles in a hospital which will prepare you better.
