mdphddermo... to qualify what I'm about to say, let me state that I'm an American who attended Med school over in Australia and have dealt with several different types of visas with the Australian goverment, but what I'm about to suggest is not gospel...
The real crux of becoming a permanent resident comes down to medical registration. If you are a foreign trained doc, you are given conditional registration meaning that you are bound to one hospital or even one attending/consultant for the purposes of your visa. This all changes once you pass a set of exams, the AMC exam (http://www.amc.org.au/exam.asp). after completion of the exam, you can obtain full medical registration and then are suitable to apply for permanent residency. I've dealt with other visas however have little experience with the PR visa and therefor little help.
