Is it possible to create positive changes in Behavioral Health as a civilian employee who can barely stand the inefficiencies inherent in the military?

There are lots of ways employees can make changes even if they aren't in uniform. There is nothing that says you must be active duty to serve on many committees or other avenues of change. Depending on what you are looking to accomplish will determine where, how, and how much can really change.

If it's "inherent" then it's going to be difficult to effect change.

Can't force people to drink the koolaid. Some will be accepting, others not. Working with the person in indemnifying areas which they are intolerant of while making the most which they accept. Not a big deal to taylor the work environment - doesn't need to be rigid unlike the military.
 
There are no changes, just the illusion of change or in the best case scenario spurious or superficial changes meant to pacify.
