Cash Only/Some Cash

As most plastic surgeons are not cosmetic only, I assume they accept insurance in most cases. However, I have heard of general surgeons who only accept cash, even though their operations are all covered by insurance. Do any plastic or hand surgeons do this for reconstructive work? How good does one have to be to get enough patients this way, or is it "well known" more than objectively "good?"
