There is no medical privacy anywhere. HIPAA is just a bureaucratic mess which causes reams of paper to be wasted every single day in doctors offices and hospitals all over the country.

And much like OSHA standards, no matter what a doctor or hospital does, they will never be in compliance because they won't have some form filled out in triplicate or not every single employee will know where the "HIPAA manual" is at all times.

The only way you can have any sort of medical privacy in this country is to pay cash for every single medical intervention or test from the moment you are born until you die. You ever file an insurance claim? Your "PHI" is in about 15 different data bases. Ever apply for life or disability insurance? 15 different data bases. Military? 15 different data bases.
