Community Pharmacist in Discharge Planning

Just wondering if anyone has any experience with d/c planning as a community pharmacist. With Medicare penalizing hospitals on readmisssion rates, it might give community pharmacists more leverage in assisting hospitals with d/c patients.

Just looking into something along these lines for my practice...

There are a few independent pharmacies trying to incorporate this model into their practice. Walgreens is on the forefront, but they are providing the service to hospitals for free in order to gain market share. I have another novel idea I'm working to to take advantage of this situation, put the pharmacist in a leading role with regard to prevention and cost savings, and also create revenue generating machine all while reducing CMS expenses.
