Constrictive population pyramids

I understand that these have to do with low birth and mortality rates and hence have an inward indentation at the bottom of the pyramid. But what would they correspond to on the demographic transition scale? Is it characteristic of developed nations or characteristic of Malthusian theory (i.e. death rates > birth rates)?
