Does anyone know what percentage of programs wait for the Dean's letter and AOA nominations before sending out interviews? For programs that wait for the Dean's letter, what is it that they are looking for? I would appreciate any input.

We didn't, but then again I was involved in this when it was an early match so Dean's letters didn't really matter at all. I still think your LOR are more important than the Dean's--unless your dean is ENT.

AOA is a bigger deal and that could definitely tilt your application one direction or other.
 
Do programs realize that some schools don't nominate students for AOA until the end of October/early November of their 4th year?

Yep, and they're aware that some schools don't even have AOA. Sucks for you as it did for me.
