Do I need to have recommendation letters from each one of my research experiences in my essay?

Some schools requires LORs from all the PIs you worked with, but it is going to vary from school to school.

Did he/she tell you why they wouldn't be able to write a strong letter?

As per mentioning it vs not mentioning it I wouldn't really be sure, a mediocre letter is not good but neither is leaving out a PI you worked with, and 4 months isn't an insignificant amount of time.

In what light are you mentioning it in your essay? Seems easier to just not talk about it much.

I don't think it is the biggest red flag in the world not to have an LOR, but might make some pause when reading your app. But doesn't sound like getting the LOR would be any better, and likely would be worse.

You'll just have to look at the individual school requirements, and perhaps be prepared to answer why you don't have an LOR/about the experience and frame it in the best light possible.
