This might be very late but... Anyone know about programs that have open spots for hospice/palliative care medicine fellowship for this year July 2024? Or would I be better off applying for the 2025 fellowship?

There are definitely openings somewhere if your goal is to find a program anywhere. Probably many.

I have forgotten the timing of match and soap, but it does t hurt to ask places. If they have open spots they may well be willing to fill after soap. Just expect an interview process like any other spot. Would call local programs if you can show genuine interest and some evidence of this (at least shadowing someone in specialty)
