Actually I am only a premed, but I was curious to know whether or not you have to start paying back your student loans when you have a fellowship?

It depends on what kind of loans you have, but if they're federal student loans then the answer is "no." Fellowship training gets you an automatic deferment on federal student loans. Interestingly, residency training does NOT automatically get you a deferment, but if you have enough loans you can qualify for a "hardship deferment." Interest still accrues on the unsubsidized loans during these deferments though.
