So I'm a little confused about the p/f curriculum and being "top" of your class. I keep hearing that it's important to be at the top of 5% or whatever of your class by a couple residency PD's (as one of the criteria for trying to snatch a strong residency) and well it's plastered all over SDN too. My Q isn't about that though (I frankly don't care too much if that's true or not...yet...more confused about the p/f system).

With a purely P/F curriculum, how does that even happen? One of the schools I was accepted to claimed to have no internal rankings with a P/F curriculum but then also referred to a specific second year as participating in a lot of EC's but also being "top of his class." How is that possible? My post may not be very cohesive because I'm kind of confused.

You still get a percentile score for exams and courses.

Unless the school is one of the very few that is true P/F all 4yrs, you'll also still have grades during M3/M4.
