For TMDSAS do we have to include the transcript request form with our transcripts?

Some of my schools are weird and won't let me attach forms with official transcripts, so is there going to be any issue with me just sending transcripts to TMDSAS without the form saying my TMDSAS ID and stuff? Can they just match our names to our application with transcripts? Or is this going to cause an issue? I've already went through hell with AACOMAS so I'd like to avoid that.
