Volunteering/working to show altruism doesn't so altruism, volunteering/working to help other people shows altruism. Stop being so concerned with how you're coming off to the adcom. You're not going to be able to trick them. If you are truly altruistic, it'll show and whether or not you did something paid or unpaid won't matter.
 
I'd be more worried about the fact that you apparently have no clinical volunteering experiences...

I would say that 100 hours of non-clinical volunteering is fine. Because you were paid for your internship you should not classify it as volunteering on AMCAS.
