Ischemic Optic Neuropathy.
Anyone have a case where this has occurred?
I read the ASA update on avoiding post-op blindness for prone cases:
1)if its gonna be long, do it in 2 stages
2)maintain blood pressure in the "normal" range
3)blood for blood, or start using colloid after a couple of liters of crystalloid
4)If you're going over 2 liter blood loss then start thinking about staging....don't see how practical this is.
5)be super "cautious" in diabetics, autoimmune folks, arteritis peeps, etc.
We ROUTINELY do long spine cases. Large volume blood loss (2 liter). 8-10 hour cases. Luckily I haven't had this happen yet.
