1) Some might encourage or require community service during the med school years, but for most it's optional. I am not aware of educational differences.
2) No. Their students match into any specialty. There is no limitation.
You'll often see these programs have their primary teaching hospitals serving low-income communities with disadvantaged patient populations. I'm at one of those schools. We have a student run health center that serves one community and I spend one day a week going primary care at a community health center. Our 2017 class matched broadly into everything from plastics, ortho, urology, ophtho, rads, etc.
