Let's say unstable a. fib (no pacemaker or ICD in situ) and the chemical cardioversion has not worked.

Are they at risk for life-threatening respiratory failure from prolonged muscle contraction after a synchronized shock?

How does it work for those who have an ICD in place and may get shocks for VT/VF when at home? Do they go into respiratory arrest?
