ONe of the questions on the Free NBME practice material

presents a woman with a fever and a seizure, and confusion.

MRI shows bitemporal hyperintensities.

LP is done. It shows 340 RBC's
leukocyte count 121 (88% monocytes)
protien concentration 78

the answer is herpes encephalitis.

why would monocytes predominate on the LP. Should there be lymphocyte predominance???
