I think you are an incredibly unique applicant. Given that you are going to have a PhD and have done well in the prereq classes so far I really don't think your undergrad gpa is going to affect you too much. I think you have proven you can handle the rigorous curriculum of med school.

Honestly, I think you are competitive at most, if all, DO schools. It's difficult for me to say what MD schools would give you a shot....I would imagine many of them will. I'm sorry I am not making a list for you but I would apply to a lot of schools and apply broadly....apply to a few top-tier med schools and then a bunch of mid to low tier schools. And apply to maybe half DO schools. All together you should have ~20 schools. That is more schools I would suggest for most people but I think some schools may write you off but that many will be very interested in you.

There are a good number of MD schools that reward reinvention. Start with:

U MA
Case
U Miami
Duke
Columbia
Dartmouth
BU
Tufts
Albany
Drexel
NYMC
SUNY Upstate
U VM
Other lower tier schools.
All the new schools, except Central MI

"Family ties" to a state mean nothing.
