I currently have around 45 hours of observation. The issue is, I only have documented "proof" for 20 of those hours. Do schools really require proof for all shadowing hours? Also, if anyone has recommendations for places to reach out to in the Chicagoland area please let me know. Due to COVID, it has been a struggle to find a place that allows students.
Well I am not from Chicago, but for a majority of the schools I applied, they never really asked for any "proof". Since you will leave contact info on your app and if they have any questions, they can call or email to ask and confirm. Unless the school specifically ask you to fill out a form or anything else, I feel like you won't need to provide documentation. If worse comes to worst, would the OT be able to write and sign something that can say they certify the amount of hours you've done?
