observership for neurocritical care or interventional neuroradiology

I'm currently a 3rd year resident in a combined neurology and psychiatry program here in the philippines. My interests include neurovascular and stroke as well as neurocritical care. after residency, I plan to take a year of stroke fellowship here in the Philippines. After which I would like to go into training abroad, preferably in neurocritical care or if possible endovascular/interventional neurology. The problem is I haven't taken the USMLEs. I would like to know if there are centers offering observerships or visiting fellowships for these fields. Thanks.
