Promotion from O3 to O4 in the Reserves - computation of prior civilian clinical experience

Trying to understand how promotion from O3 to O4 in the (Air Force) Reserves works if you join as a fully residency trained physician with some clinical experience (but not enough) and therefore your initial rank is O3. For your subsequent promotion from O3 to O4 in the Reserves will your (pre-reserve) clinical experiences be considered at all or are things 'reset' and only the 'in-reserve' time will count for the subsequent promotion to O4? Thanks s lot!

Your service would compute into your date of rank and would be taken into account for the next rank.
Essentially, you would come in as a "senior" captain.
