Do you recommend for the patient to hold the albuterol 1 to 2 inches away from lips or to wrap lips around the mouthpiece? I've heard both ways. Which way is better?

Also, do you recommend a toilet paper roll as a spacer if the patient cannot afford a real spacer?

That 1-2 inch away technique for albuterol inhalers should have disappeared when the CFC inhalers were banned. The HFA ones are not strong enough for that. You need to hold the inhaler in your mouth.
I would not recommend a toilet paper roll as a spacer because you can't get a good seal on it. You'll loose a lot of the drug. The cheapest spacer we have retails at about $15. WI medicaid covers one spacer every 6 months.

CFC produced a stronger, cooler, worse tasting spray. I'm not sure if the puff isn't strong enough to bridge the distance - but with HFA, it is much easier to put your mouth around it. Always encourage the spacer though - it produces the best form and helps the kiddies judge the proper inhalation speed.

make sure you let them know the cap on the inhaler piece has to come off........
 
squirt the 3ml vial through the nasal...
