Reference Letters PLEASE read...ned help

Hello everyone, I just spoke to my Ochem professor and she is willing to write me a reference letter and she asked about the format for which to write it? Does anyone know what the format should be for reference letters?

Also, I am going to shadow my optometrist in a few weeks and I need a reference letter from him and when I spoke with him a few days ago and he was not sure how to write it. Did you optometrists type your letters of rec out and send them to the school or did you mail the letters of rec to the schools yourselves?

Where on Optomcas? I am not seeing anything on that website for letters of rec.
