I was hoping someone had that graph (and data source) that showed the cost of immunotherapy (might have been pembro alone) versus the entire cost of radiation as a field. that was circulating around here not too long ago (could not find searching)

If rad onc were following this financial Moore's Law that other cancer therapies are, then the entire planet's governments would be bankrupt by around 2050. There'd be no money for food, clothing, or shelter... only cancer treatments.

Haven’t you figured this out yet. It’s not because you are charging too much that makes you a target for CMS. It’s that you’re charging too little! So less vested interests and less lobbying. If nobody has an interest then nobody cares if you get cut or if you exist at all.

Whatever happened to scarb anyway? Is he with KHE in the Cayman's?
If you go into a bathroom, turn off the lights, look in the mirror and say "breast field-in-field is IMRT" three times, legend has it that both will show up behind you, an hour into a debate about it.

I heard that Scarb and KHE have a linac on a semi, providing radiotherapy services throughout rural America with a mobile linac. One drives and the other contours. They do mobile visits on the road, park, treat, rinse repeat. They stop for lunch, going through the ports and CBCTs. The patients are so happy that cancer care comes to them. There are lines of them at each stop. After a 16 hour day of treating, they crack open Bud heavies and eat chili that they make roadside. The sit around a campfire to keep warm. They tell tall tales and spin yarns, talk about "what coulda been" and do peer review in intense "Murder Board" style. There are occasionally fist fights after harsh words that end in tears and hugs and promises to do better. They are basically the modern day cowboy, but instead of rustling up steer, they cure cancer and on Friday, they empty the dumpster truck of money and convert it to various crypto currencies.

Will they post openings on ASTRO, or word of mouth?

I'm going to start spreading the rumor that they're microchipping people with their tattoo ink.

Hard to say if KHE will believe it himself and leave the practice. Probably 50/50 at worst.
