under what circumstances have you encouraged patients to seek a second opinion from another pcp

Happened a couple times randomly. 1 of those times it happened to be in a niche I'm interested in. Otherwise it would not be reasonable to provide subspecialized 2nd opinions.

When a patient disagrees with management options I provide them with, they are all given options for a referral to a specialist for a second opinion. At any time the patient can establish with alternative PCP if he or she so chooses to.

If a patient sees another primary care physician without discussing it with me first, that's a transfer of care, and they can't come back.

I won't do second opinions for other PCPs, either. If the patient wants to transfer care to me, fine. If not, go elsewhere.

What if it's in a niche area that you're known to excel at?

The only exceptions I've made over the years were for a handful of patients who wanted to see somebody in our group who did bariatrics or OMM. Since we all (me, the patient, and the other physician) discussed it beforehand, I allowed it. It still ended up being awkward, however.

If someone wants a PCP who does something I don't do, it's just easier to transfer as opposed to seeing us both.

That's what I do for those who want OMM.

Do y'all consider the NDs as PCPs and drop care then?
If NDs aren't an issue in your state, prepare for it.
In my parts they are dosing out zyprexa, transgender hormones, mood stabilizers, etc
