Case: left elbow synovial sarcoma, resected after neoadjuvant chemotherapy. Initial tumor size <5cm.
Negative margins.
But very large scar. If totally included in the first 50 Gy --> whole left arm in field

Is it justified not to cover the whole surgical scar and be selective ?

I probably would not radiate. Follow closely with MRI q3 and if recurs do preop dose to a much smaller volume before re-excision.
