Inherited a patient on lamotrigine ER 150 mg/day for mood stabilization. Due to cost, patient wants to switch to immediate release formulation. Is it possible to switch immediately to 150 mg/day of IR? My only concern is the peaks are higher in IR, and this could increase the risk of SJS somehow. Haven't seen any literature on switching from ER to IR. One thought I had was to go to 100 mg/day x 14 days, then increase to 150 mg/day. Thoughts? Thanks in advance.

Don't take my word for it but I bet this is pretty similar to Depakote er and ir.dosing is 1:1.

This article suggests you can switch directly without change in dosing or
