Do top research schools show any disfavor to applicants who express a strong interest in going into a primary care field or working in public health, with no research ambitions? Are they solely interested in pumping out top notch academics?

Not necessarily. Research is highly valued, however, and most do have explicit missions of churning out academics.

Either way, if you have no interest in research/academia, it probably doesn't make sense to apply to "top" research schools - unless your interests in public health/primary care is supported by the school (Columbia comes to mind).
