Hey guys, which GPA counts for US dental school?

I did the course-by-course evaluation and received a 2.6 and 3.46

Also, what documents do I need to send to apply for the Canadian exam and its cost?

Have you taken the NBDE Part1 and or 2 test? If not, you need apply the dental pin first. Then register for test.
By the way, what documents did you submitted for course-by-course evaluation?
