First of all, stop tutoring. You likely have a lot of non-clinical volunteering but you don't describe it or the number of hours.

If you have been working with immigrant/refugee services, look for schools that may have clinical opportunities where you can volunteer or rotate there. Boston University comes to mind (Boston Center for Refugee Health and Human Rights (BCRHHR) | Psychiatry). There are also AMMSA chapters at BU and at UNECOM (which I think you should strongly consider given your MCAT range).

What medical schools have you visited, and have you made connections with students at schools on your wishlist?

You are not competitive for MD schools with a MCAT of 499 but you could receive interviews at some DO schools.
I suggest these schools:
UNECOM
LECOM (all schools)
PCOM (all schools)
WVSOM
UP-KYCOM
VCOM (all 4 schools)
NYIT-AR
ARCOM
WCU-COM
ACOM
LMU-DCOM
UIWSOM
BCOM
Noorda-COM
KHSC-COM
ICOM
Touro-Montana
RVU-Montana
Orlando (when it opens)
