Worthwhile for Canadians to write USMLE (Step 1)?

Current MS1 in New York (I'm Canadian), planning to go to go back to Canada for residency - USMLE Necessary?

Only if you will need it for those fellowships or you plan on working in the U.S. one day. I believe you need USMLE for fellowship programs so yes you want a high score.
