Write the patient's weight in the corner of the script and then figure out the amount of drug for the given weight and then the volume of a certain concentration needed to be given to acheive that amount of drug

Eg:
Patient weighs 25 kg. Tylenol comes in several concentrations, one of which is 160 mg/5 ml (5 ml = 1 teaspoon). Tylenol is given 10-15 mg/kg PO(by mouth)/PR (per rectum) q6-8 h prn fever. Say we give 13 mg/kg. Then the patient needs 325 mg for each dose. Divide 325 by 160 and multiply by 5 to get the volume of each dose (about 10 ml or 2 tsp).

Sig: take 2 tsp PO/PR q6/8 h PRN fever or pain
Disp: 500 ml
Refill: 3 (three)
If you are giving a drug for a certain duration (eg, antibiotic for 10 days) you can use "QS" in "disp" line for quantity sufficient and the pharmacist will give her enough for 10 days.
Eg:
Amoxicillin 400/5ml
Sig: take 2 tsp PO q12h x 10 days
Disp: QS
In this case the pharmacist will make sure the pt has enough for 10 days (ie, at least 200 ml).
