I'm an Illinois resident, and I'll definitely be applying to my state school.

When you apply to the University of Illinois Medical School, are you effectively applying to all of its campuses (UIC, UIUC, Rockford, Peoria)?

I searched this topic, and found the following:

once accepted, they ask you very specifically which campus you'd like to attend. Whether there is space left at that campus, however, is not guaranteed.
Can anyone confirm the accuracy of that statement? Does anyone have any more information on the above policy? It seems strange to me, but I suppose you never know...

Yes, that's accurate.
 
When you receive an acceptance to UIC, you are asked to return a postcard ranking your choices of the four campuses. Chicago, which gets 170 of the 300 accepted students, fills up first, generally in January. If you don't like the campus you are assigned to, you may write a letter petitioning for a change should a space open up, giving your reasons. Very few students spend four years at Urbana, unless they are in the MD/grad school degree of some kind programs. The total here is 30. Of the remaining 100, half go to Rockford, and half to Peoria after spending their MS1 year in Urbana. A few of the Urbana students choose to decompress their MS1 year and do the same coursework over two years. On rare occasion someone gets accepted late in the cycle and gets assigned to Chicago, presumably because another student just gave up their spot, but also possibly because they like to balance the characteristics of each class in some way.

Also good to know, it doesn't matter which campus you interview at, so take the first available spot wherever it is, or however close it might be if convenience is important.
