I don't think much has changed in last few years
The jobs in smaller town 90 minutes or more outside of major cities remain well compensated, flexible work weeks (3-4 days in office not unreasonable) and having worked / locumed in places like this, job itself tends to be great - just if you can manage to live in the smaller towns.
City and suburb and exurb jobs are less well paid, less flexible and also less options available. Many are still 5 days a week with no WFH (mine included). Some hospitals, despite this, still limit time off.
Pay attention to benefits, retirement match, health care. These vary widely and can change the bottom line compared to just reviewing salary alone.
If I were a young man graduating now and had no kids, I'd go to one of these small towns and bank for 5 years, maintain connections to cities/suburbs I was interested in and then proceed to move when kids are in elementary school. If not having kids, enjoy that 3 day in person work week, make a ton of money, vacation widely, have a summer or winter home and enjoy taking care of small town folks. I have not only found the patients gratifying to work with, but the staff tends to be excellent (when they can be found). Typically, it's a person that wants to be in that town and is pretty much stuck because it's likely the only radiation oncology job within many miles.
I work harder and make less money in absolute dollars then I did 10 years ago. That is not uncommon. Accounting for inflaysh and cost of living, definitely earning less.

ABR Consulting LLC, a Service-Disabled Veteran-Owned Small Business (SDVOSB), in conjunction with Cancer CarePoint, is recruiting an experienced, board-certified Radiation Oncologist for an anticipated long-term engagement supporting the Baltimore VA Medical Center Radiation Oncology Clinical Care Center in Baltimore, Maryland. There is an IMMEDIATE need for a radiation oncologist and the suitable candidate would ideally be ready to commence work in fall 2026 (but we are happy to discuss a suitable timeline for the right candidate).
This is an onsite independent-contractor opportunity expected to range from 0.75 to 1.0 FTE, with a guaranteed minimum workload equivalent to approximately 1,560 hours annually (0.75 FTE). The required workload may flex up to 1.0 FTE (2080 hours) annually depending on VA needs.
Compensation for the independent contractor position will be determined based on the physician’s skill and experience, and will range from $425,000 - $500,000 annually for a 0.75 FTE arrangement, and $550,000-$650,000 annually if VA needs flex to a 1.0 FTE arrangement.
The physician will work alongside VA-employed Radiation Oncologists and an established multidisciplinary oncology team. Current clinical support includes five radiation therapists (one Chief RTT and four Staff RTTs), one dosimetrist, two medical physicists, three nurses plus a CNA, and administrative support. Dedicated onsite dosimetry and medical physics support will be available.
The Baltimore VA provides a diverse but manageable Radiation Oncology practice that offers most forms of modern external beam photon radiation therapy (IMRT/VMAT, 3DCRT, IGRT, SRS, SBRT, and electrons) but does not perform any brachytherapy. The VA replaced their linear accelerator and CT sim in 2025, and thus best data for assessing one full-time equivalent has come from 2024, when the VA was staffed by 1.5 full-time physicians who shared the following annual workload:

Candidates must meet the following requirements:
- Board certification in Radiation Oncology by the American Board of Radiology (ABR) and maintenance of certification throughout the engagement.
- Current, full, and unrestricted license to practice medicine in any U.S. state, territory, Commonwealth, or the District of Columbia.
- Minimum five years of documented post-residency Radiation Oncology experience.
- At least three years of continuous Radiation Oncology work experience within the previous five years.
- At least five years of post-residency experience involving consultations, CT simulation, treatment planning, treatment management, and follow-up care.
- Experience with Varian linear accelerators (Edge or equivalent).
- Experience with IGRT, including Cone Beam CT.
- Experience with Eclipse treatment planning and ARIA record-and-verify systems, including 2D/3D conformal radiation therapy, SRS, SRT, SBRT, and IMRT/VMAT.
- Experience with Siemens SOMATOM g😵pen Pro CT Simulator or equivalent.
- Current BLS and other required VA clinical training/credentials.
- Current NPI.
- Current DEA registration in Maryland will be required.
- Ability to successfully complete VA credentialing, privileging, background investigation, and mandatory training requirements before providing clinical services.
- Ability to provide onsite clinical services at the Baltimore VA Medical Center.
- Fluent spoken and written English and ability to communicate effectively with patients, families, referring physicians, and multidisciplinary clinical staff.
- Prior experience practicing within the VA or another Federal healthcare environment.
- Experience with VA CPRS.
- Familiarity with VA credentialing, privileging, quality-management, peer-review, and documentation requirements.
Good point my understanding is that they've had a hard time implementing the measures of the Elizabeth Dole Act because of (no surprise here) an inability for the VA to determine how to do it:

Detroit would only give 10%. And they make you stay on site for 40 hours. You leave half hour early, you make it up next day. I found that infantilizing. Especially with how little workload. I think for 2 fte in Detroit they have like 20-25 on beam.

I was thinking the same thing but I noticed it says independent contractor so this may be some odd 1099 setup where the taxes and benefits are different

there's probably a mandatory salary and then a va.
doubt the salary comes from the va. Sounds like someone bid on the contract and now hiring for the position. Not sure how stable something like this is. Va may decide to hire their own doc someday.

The VA can pay up to 150% to specialty, so instead of 400 max (highest salary cannot exceed POTUS salary), it can be up to 600k. But this one appears contracted. It’s depends on the region and the need. I know that PR and Miami VA have that 150% allowance for RadOnc
Careful with these VA jobs through recruiters. I was talking to one a long time ago and they were really pushing the lifestyle even though the money was low. I finally said I wasn’t interested and was walking away and they said “well we could throw in benefits if that would help?”
