I think each school has a different procedure and each has a different interest in filling the current class. I would talk to them earlier, rather than later. Bear in mind, some schools may be totally unsympathetic to your wanting to defer a year (just brace yourself and make sure you have an extremely compelling reason for wanting to defer).

Contact the schools now, before May 15, as this may play a role in which school you choose. Depending on how over -enrolled they are, they may be happy to let you defer or they may be less enthused.
At this point, I don't see why you'd have to withdraw from waitlisted schools. Even an applicant who selects a single school on May 15 is not required to withdraw from waitlisted schools.
The MSAR will tell you that there are 12 US schools that don't accept requests for deferral, but not how likely the others are to grant it. Schools vary in either having a liberal policy, to requiring a very, very good reason for the request. If you are shy about calling schools, you might first check in SDN's PreMed Allo and Allopathic School Specific forums to see if the information you need is there.
