I'm interested in seeing how competitive the integrated plastics programs are. (I've heard, by far, they're the toughest residencies to get into)

My Step 1 is pretty decent (240+) but I've got no honors thus far in 3rd year (including surgery, ugh) just high passes.

Worst of all, my school doesn't have a plastics department let alone an integrated plastics department which means no mentor, no 4th year rotation in plastics planned yet.

Should I be looking at another field with all this stacking up against me?

(My backup at the moment is general surgery. Which, interestingly enough, affords me another chance to try for plastics down the road 😀)

240 will get you in the door for a plastics interview. Many programs appreciate the fact that your school may not have a plastics department. For that reason, you need to do several away rotations. These rotations will accomplish several goals...first and foremost, they will expose you to plastics and give you a foundation of knowledge and experience. Secondly, it will give you access to attendings for LORs. Third, it will give you an opportunity to shine and dispell any doubts about your application. Lastly, even if your program doesn't have a formal plastics department, you should try to get into any type of clinical or basic science research...case presentation at the very least. Never give up...apply to plastics with GS backups if you're worried.
