1+3 Year Residency Questions

I've been doing a lot of reading up on certain specialties that require 4 year residencies, where the first year is always an independant 1 year internship, followed by starting the residency as a PGY2

My main question is this: When match time comes around do I just try to match into my internship and then a year later try to match into my desired specialty?.... or is it that I match into both during one match?

I'm asking because I've become interested in PM&R and most residencies that I've looked up are the 1+3 type.

Another question that complicates the issue is if I want to do an Osteopathic Traditional Rotating Internship followed by an Allopathic residency. Curious if this is an impossible route or just greatly complicates things. Thanks a lot in advance for your advice.
 
From what I understand you apply to an internship and specialty at the same time. Some specialties have a built in intern year. So if you match Ortho at Worlds Best med school you would also be in the intern program.

Most of the time you can get your internship year to count as your osteo requirement. The inverse is true in some specialties, ie you can do a Osteopathic Traditional Rotating Internship instead of a intern year. I've heard of some specialties not the osteo year so you may want to check that out.
 
I noticed in the 2004 PCOM match list (yes it exists) that 11/12 graduates who matched into PM&R have two sites listed; one for their internship location and one for their PGY2-4 location. This leads me to conclude that they matched into both at once. How that was done, I haven't a clue.

The one graduate who had only one site listed matched into U of Virginia's program, which accepts 2 PGY1s annually.

It also seems that most have entered ACGME accredited residencies following osteopathic rotating internships.

I think the match program will match PGY2 and PGY1 programs separately.

A related question... does this mean PGY1s that didn't match into a 2nd year program will go through the NMRP again in the fall of his/her internship year? Or can they only scramble in the spring of their internship year?
 
I do understand that they match separately, however does that mean that one can participate in the osteo and allo match in the same year if one is used for matching the residency only and the other for internship only?

If you're applying for one of the allopathic residencies that do not have an associated internship year (such as anesthesiology or radiology), you can apply for an osteopathic internship through the osteopathic match, and the residency though the allopathic match. (You match one year ahead for these residencies.)
