You're too early to be planning this.

However, the "staying close to significant other" thing is the only reason I see for you to do a gap year.

Focus on rehabbing your GPA, get more volunteering, and reassess where your relationship and app preparation stand a year from now.

clearly shows that you're nowhere near ready to start med school.

You're 1.5 years into undergrad and you think you're ready to start med school? That shows a mind-boggling amount of hubris and a total underestimation of the demands of med school. Add in the fact that you're saying this immediately after a significant drop in your academic productivity, and it raises concerns for a stunning lack of insight.
