It's a small advantage. 4.0 from Podunk State in Underwater Basketweaving is better than 3.2 from Caltech in Astrophysics. You might get cut a little slack if you're from a prestigious school, but not much.
 
Going to HYPSM vs going to other top 20 undergrads won't matter at all. The only time it might matter a tiny bit is, for example, that a 4.0 at Harvard carries more weight than a 4.0 from a community college, but either way your MCAT will provide a standardized metric for your academic abilities. No adcom is going to automatically turn someone down for attending an obscure college if they scored 520 on their MCAT or something. I have met several people on the T20 interview trail from smaller schools or even colleges that I had never heard of before, so they're clearly doing alright for themselves despite not attending HYPSM.
 
The advantage of going to HYPSM will be very slight in comparison to going to another T20 school. It will more so be if the adcoms/interviewers also went to HYPSM. If the adcoms/interviewers did not go to a T10 school then they likely don't think it matters. If they did then they might be a little more impressed if you did, but it is certainly not a large factor. T20 schools are very well respected and very well represented in med school matriculants.
